Micah House Miniwalk Registration

To register as a walker in the MICAH HOUSE WALK, please complete the form below and click submit to register your form with the office.  You will then have the opportunity to pay safely and securely with Paypal.  Paypal lets you use your credit card or your Paypal account.  Or, you may send the Temple office a check payable to "Micah House".

By hitting "submit" above, I agree that I have read and accept the following statement:  I have full knowledge of the risks involved with, and undertand that I could be injured during, the Micah House Walk.  I agree to assume all risks of such injury. I unconditionally release and discharge Micah House and all other entities and persons involved with the Micah House Walk from any and all claims, damages, and costs that may arise, directly or indirectly, from my participation in the Micah House Walk.  If I am the parent or guardian of a child participating in the Micah House Walk, I give consent to my child's participation and grant the same unconditional releases on his or her behalf, as described above.
